Enabling and Active Support
We actively enable the individuals we support to achieve their personal best through Active Support and ensuring Enabling Environments.
This enables them to maintain and develop new skills and maintain their independence as much as possible
Supporting Discharge and Life Plans
In supported living we support people who often have complex needs and may have seen their previous placements breakdown - to create a pathway from hospital into the community with great outcomes and a much reduced length of stay.

Personalised Home Care Plans
At Care Consult, we develop customised care plans tailored to each individual's needs, preferences, and health requirements. Our dedicated team works closely with families and Multi disciplinary teams to ensure that every person receives the appropriate level of support, promoting independence and well-being in a warm, supportive environment.
