Service user A was identified as a long stay patient in a low secure hospital for adults with learning disabilities for over 11 years. Being detained under the Mental Health act and having a diagnosis of learning disabilities and personality disorder meant that service user A was labelled as unsuitable to place in the community. This was despite a very long period of therapy and no risk behaviours being presented and fundamentally her overstay in hospital was not in line with the Transforming Care Programme and agenda for people with Learning Disabilities (2015).
A referral was made to Care Consult ID and following a series of comprehensive assessments, robust recommendations including service user A being supported in the community on extended section 17 leave, an offer of support in the community was made to the service user , family ,treating clinical team and commissioner.
A 12 weeks transition plan was collaboratively formulated which included initial day trips and then overnight stays to the service users chosen community setting . Weekly reviews were held and sign off on key achievements of the transition plan was completed collectively.MDT input and support remained integral with involvement in the formulation of the community support plan and training directly facilitated to our staff team. This included service user specific training on the Positive Behaviour Support and their specific needs.
During the transition the service user was supported to achieve most of the key principles of the Supported Living REACH standards and successfully gained a tenancy agreement for a bungalow that they fully redesigned and choose all the furnishings and decorations. Service user was successfully supported to a open bank account which was a great achievement for her and to register for all her household bills; she was able to access most community amenities ahead of her transfer, which really gave her a positive outlook.
Service user A was successfully commenced the extended section 17 leave after the 12 weeks all set to start a new life and celebrated her first Christmas in six years with her family in her bungalow. She remains very focused on meeting her life goals with the support of Care Consult Staff whom she has established very good support relationships with.
Service user A is awaiting the outcome of Deprivation of Liberty Safeguards (DOLS) application and imminent discharge off section. A good outcome thus far for a long stay in patient!
