What is Supported Living in the community ?

Supported living services offer care and assistance for those living in rented accommodation or in their own homes. Under a supported living arrangement, personal care and accommodation are provided under separate contractual agreements. Housing is often provided through the local authority or a registered housing association, and funded through the individual's benefits or earnings. Support is delivered by a care provider, like Care Consult and funded by the local council and in some cases the NHS.


Engagement, involvement, inclusiveness and enablement  are themes that run throughout all aspects of our work. We maintain an ethos of individual development through experience with the aim to support each individual in achieving independence and appropriate community access, presence and participation. This will include a range of developmental and therapeutic support and opportunities. Individuals will also be supported to take up meaningful employment in the community and or attend local colleges for vocational studies of their choice

  • Rent and accommodation- we do not pay for individuals 
  • Food and drink - we will support with shopping 
  • Furniture - unless in furnished accommodation, tenants are required to furnish their own rooms and contribute to any communal areas
  • Utility bills
  • Maintenance and repairs - where not covered in the tenancy agreement
  • Activities - you need to pay for any activities you participate in
  • Transport - we can support you to use public transport if required
  • Any specialist equipment - we can support you to contact services such as Occupational Health
  • Personal items, such as clothing or personal care

 We engage by involving people who use our services and their families, and care managers, in creating their own unique person-centred plan that takes into account their individual abilities, limits, wishes, preferences, ambitions, aspirations and needs (including health and mental health needs), to enable them to live

We work with a number of registered housing providers and Supported Living Gateway organisation. This enables and facilitates ensuring that homes are affordable and individually tailored.

We understand that highly skilled care and support is frequently required to support a number of clinical, therapeutic and community health professionals. We have experience of working alongside extensive multi-disciplinary teams and managing complex dynamics .

Our Staff

Our staff team comprises of experienced

Management 

Learning Disability and Mental Health Registered nurses Support workers 

Independent Consultant psychiatrist

Occupational therapist. 


· Staff support will be arranged on a needs led basis

· Staff will be required to work on a rota and named key support worker system, to meet the individual needs of the individual on a daily basis

· Additional staff will be arranged to meet any additional needs

· Our staff have experience of supporting individuals in the community. We support individuals to increase their independence and to ensure that this is a home for life, should they wish it to be

· The management and staff team receive comprehensive training, including all mandatory training and bespoke training specific to the needs of the residents

· All staff are subject to a robust recruitment process, which includes an enhanced DBS prior to commencing our employment.

Standards of care

We provide responsive and professional service at all times, as well as we ensure confidentiality to both our service users and our candidates. 


We will never compromise on quality or provide staff that do not have the skills to match our service user

requirements.

    2. Assessment is arranged and undertaken


    3.. Assessment report is completed and shared with the referer/funder


    4. An initial proposed support plan and funding information is shared and package is agreed


    5. Transition is agreed with the individual and funder, following confirmation of acceptance of the placement


    6. Initial Admission support plan is agreed and admission is completed.
